1. Entity Responsible for Collecting and Using Personal Information
The information provided through online consultation is directly received and managed by the "Dental."

2. Right to Refuse Consent and Disadvantages of Refusing Consent
Applicants have the right to refuse consent for the provision of personal information, etc. (In this case, online consultation application will not be possible.)

3. Items of Personal Information Collected
Required items: Title, category, applicant's name, mobile phone number
Optional items: Email address

4. Purpose of Collection and Use
Delivery of Online Consultation

Chapter 1 - General Provisions

Article 1 Purpose
This agreement aims to establish and regulate the conditions and procedures for using the services provided by Seoul Dental Hospital (hereinafter referred to as 'Seoul Dental Hospital') in accordance with the Telecommunications Business Act and its Enforcement Decree.

Article 2 Effectiveness and Changes of the Terms
(1) The terms become effective upon public notice to the users.
(2) Seoul Dental Hospital may change the terms in case of changes in circumstances or important business reasons, and the changed terms shall become effective in the same manner as mentioned above.

Article 3 Regulations Not Specified in the Terms
If matters not specified in these terms are governed by relevant laws and regulations, those regulations shall apply.

Chapter 2 - Membership Registration and Service Use

Article 1 Establishment of the Usage Agreement
(1) The usage agreement is established by the acceptance of the user's application by Seoul Dental Hospital and the user's agreement to the terms.
(2) Those wishing to join as members and use the services must provide the personal information requested by Seoul Dental Hospital.
(3) When Seoul Dental Hospital accepts the application for use, the following matters shall be notified to the user
① User ID
② Service start date
③ Other matters deemed necessary by Seoul Dental Hospital.
(4) Seoul Dental Hospital shall not accept applications for usage agreements that fall under any of the following
① When applied using another person's name.
② When the application form for the usage agreement contains false information or false documents are attached.
③ When applied with the intent to disrupt public order or morals.

Article 2 Service Use
(1) Service use is available 24 hours a day, year-round, unless there are special operational or technical issues at Seoul Dental Hospital.
(2) The usage hours in paragraph 1 may not apply on days or times designated by Seoul Dental Hospital for regular maintenance or other necessities.
(3) Even after joining as a member, access to certain services may be restricted based on the user's age.

Chapter 3 - Responsibilities

Article 1 Obligations of Seoul Dental Hospital
(1) Seoul Dental Hospital shall ensure that the user can use the service on the specified start date unless there are special circumstances.
(2) Seoul Dental Hospital has the obligation to provide services continuously and reliably as stipulated in these terms.
(3) Seoul Dental Hospital shall not disclose or distribute the user's personal information to others without the user's consent. However, this does not apply when there are requests from relevant national authorities under telecommunications-related laws and other applicable laws.
(4) If Seoul Dental Hospital deems the opinions or complaints raised by users to be valid, it must process them immediately. However, if immediate processing is difficult, it must inform the user of the reasons and the schedule for handling the matter.

Article 2 User's Obligations
(1) The user is responsible for all management related to their ID and password.
(2) If the user's ID is used fraudulently, the user must inform Seoul Dental Hospital of this fact immediately.
(3) Users must comply with the provisions set forth in these terms and relevant laws.

Chapter 4 - Termination of Contract and Service Usage Restrictions

Article 1 Termination of Contract and Usage Restrictions
If a user wishes to terminate the usage agreement, they must personally select the personal information change/cancellation menu online to apply for termination.
Seoul Dental Hospital may terminate the usage agreement or suspend service use for a specified period without prior notice if the user engages in any of the following actions
(1) In violation of public order and morals.
(2) Involved in criminal activities.
(3) When the user plans or executes service use for the purpose of harming national interest or social welfare.
(4) If the user uses another person's service ID and password fraudulently.
(5) If the user damages another person's reputation or causes them disadvantage.
(6) If the same user registers under multiple IDs.
(7) If the user harms the service or hinders its proper use.
(8) In violation of other relevant laws or the terms established by Seoul Dental Hospital.

Article 2 Procedure for Lifting Usage Restrictions
(1) If Seoul Dental Hospital intends to impose usage restrictions under Article 23, it will notify the user or their representative in writing or by phone of the reasons, date, and duration. However, this does not apply if Seoul Dental Hospital deems it necessary to urgently suspend usage.
(2) Users or their representatives who receive notice of usage suspension under paragraph 1 may file an objection if they disagree with the notice.
(3) Seoul Dental Hospital may temporarily suspend usage restrictions for a period necessary to confirm the objection filed under paragraph 2 and shall notify the user or their representative of the result.
(4) If it is confirmed that the reason for the usage restriction has been resolved during the suspension period, Seoul Dental Hospital will immediately lift the restriction.

Article 3 User's Posts
Seoul Dental Hospital may delete any content posted or registered by the user within the service without prior notice if it determines that the content falls under any of the following
(1) Content that defames or slanders other users or third parties, damaging their reputation.
(2) Content that violates public order and morals.
(3) Content deemed to be related to criminal activities.
(4) Content that infringes on the copyrights or other rights of third parties.
(5) Content that violates other relevant laws or regulations established by Seoul Dental Hospital.

Chapter 5 - Changes to the Contract

Article 1 Changes to Contract Terms
(1) Users must change or terminate the usage agreement via phone or through the service/browser when they wish to modify the content or terminate the service.

Chapter 6 - Provision of Information

(1) Seoul Dental Hospital may provide various information deemed necessary for users during their use of the service via email or postal mail.

(Effective Date) These terms take effect from April 12, 2012.
▷ Membership registration is based on personal registration, and any false registration will result in expulsion. Any damages arising from this will hold the false registrant legally responsible.
